When students first apply for course, they are taken through a brief questionnaire called the Prescreen Questionnaire. This questionnaire helps us determine the appropriate forms to send you in addition to our standard Welcome Packet. The Welcome Packet includes an Enrollment Forms Packet, a Policies Packet, and Information for Your Course. You will also receive Course Information documents which include a Course Description, Travel Information and a Clothing and Gear List. In addition to the standard Welcome Packet and the Course Information documents we will send any Supplemental Questionnaires that identified as pertinent during the Prescreen Questionnaire.
For your convenience we have provided links to all available Supplemental Questionnaires below. These are only required if they have been assigned to you in the email with all of your Welcome Packet materials. The Participant Forms must be filled out by the applicant (or their parent), but the Physician/Counselor Forms require information from a third party, usually a medical or mental health professional.
All of the Participant Forms can be filled out electronically by typing in the gray shaded areas and attached by e-mail. You must save the data and then attach the form(s) to e-mail. You may also print the form, write out all of the answers to the questions and fax (see below) the form to the Assessment Department at Outward Bound Wilderness.
If any Physician/Counselor forms are needed, they must be filled out completely by the participants’ physician, counselor or other third party reference. All of these forms must be printed, filled out completely, signed, dated and returned to the Student Assessment Department at Outward Bound Wilderness by fax.

*Note: For the Asthma, Counseling, Diabetes, and Seizure forms there are both a participant and a physician form.
